Confined Space Entry - OSHA CFR 1910.146 Confined Space Entry Procedures - For Manholes and Lift Stations
Manhole safety is a critical part of JTV's daily operations. Many of JTV's crew members have successfully completed a 40 hour approved training course. Other JTV crew members have completed a 10 hour OSHA safety training course. A three man team is the minimum number of persons required for worker to safely enter and work within a manhole. The manhole entrant must wear a safety harness attached to an approved tripod with a anti-fall safety winch. A second worker must be stationed at the tripod at all times when a worker is in the manhole. A third person must oversee entire process with the ability to contact rescue personnel in the event of an accident. All persons involved in a Permit Required Entry must be fully trained & certified (40 Hour OSHA Approved Training). Confined Space Entry is a common work practice for JTV crews when installing cured-in-place pipe liners, chemical grouting and CCTV Pipe inspections. To maintain OSHA certification requirements, crew members must attend an 8 hour refresher course every year.
All manhole entries require the issuance of a Confined Space Entry Permit. The entry permit is completed by a JTV supervisor and lists the persons authorized to enter the manhole, current manhole atmospheric readings and local emergency response phone numbers. A multi-gas detector is used to obtain manhole atmospheric readings to be recorded on the permit and updated periodically. A gas meter must be worn at all times by the entrant when in the manhole. The gas meter is required to sample & display:
1. Combustibles (Flammables)
Methane
Propane
Gasoline
2. Oxygen - Deficiency and Enrichment
3. Toxic Gases:
Hydrogen Sulfide
Carbon Monoxide
Toxic Hydrocarbons

Toolbox Safety Meetings
Every day, for each worksite, JTV field crew have a Toolbox safety meeting. The meeting is held to review and discuss the projected days' work and assign duties to each employee. On some projects, a second Toolbox meeting is held midway through a work shift or when the work day extends into overtime. Confined Space Entry permits are typically completed during Toolbox meetings. JTV's supervisors plan & conduct Toolbox meetings and determine if additional Toolbox meetings are required throughout the day. A second Toolbox meeting is always held if additional or relief workers are added to the workforce.
At each meeting, all crew members are reminded that safety is paramount and crew members have the ability to stop work or shut down the jobsite when a safety issue is apparent. Crew members are also reminded about the safety of others, to be away of unwanted persons entering the worksite.
Surface - Traffic & Passersby Safety
JTV, Inc. recognizes e their responsible to provide safe working conditions for their employees but also for the safety of the general public. Detailed traffic detours are established before any work is begun. Barricades to prevent persons from entering the worksite are also established. JTV , Inc. is positioned at the worksite to minimize traffic disruption in and around the worksite. In some cases, directional arrow boards and /or message boards are used within the traffic detour plans. Municipalities are notified about pending traffic detours and possible conflicts with their emergency vehicle response routes.
Continuous Training
Additional employee training is provided whenever new equipment is purchased or when new types of resins or epoxies are used. Some training certificates require annual or bi-annual refresher courses to maintain current certifications.
DOT Training
JTV owns and operates a fleet of heavy highway trucks regulated by state and federal DOT regulations. Drivers must self inspect all vehicles for possible mechanical deficiencies before driving any vehicle as well as maintain personal drive logs. All drivers attend quarterly meetings to get updates on the latest regulation changes and review of driver logs.
JTV, Inc. has an additional internal driver safety policy that requires all drivers to completely walk around a vehicle before driving a vehicle as an additional safety measure. Drivers must perform this additional safety check to check for equipment malfunctions and for any obstructions or persons that may be in the way of a moving vehicle. JTV, Inc has begun installing backup cameras on vehicles as an added safety measure to assist drivers.

EMR -Experience Modification Rate
Experience Modification Rate (EMR) has strong impact upon a business. It is a number used by insurance companies to gauge both past cost of injuries and future chances of risk. The lower the EMR of a business, the lower worker compensation insurance premiums will be. EMR numbers are also used to determine the safety record of a company, proving the company has a track record of work without worker injury. JTV, Inc. currently has an EMR record of less than one, an excellent rating.
